A Letter to You:
Dear Outpatient Provider
Thank you for your interest in Centennial Peaks Hospital. We have a wide range of services for patients ages 12 and above
and provide treatment for both mental health and substance abuse issues.
We also treat patients with co-occurring disorders.
Therapists, psychiatrists and other
professionals from the community refer their patients to us for our risk assessment which
assist them in determining their patient’s level of risk to himself/herself
and to others and to provide appropriate care. Generally, we
are able to schedule a requested appointment for an assessment
for the same day we receive the referral call.Our assessment department conducts risk assessments to determine level of care (inpatient or outpatient)
and program placement. These assessments are conducted by masters
level, licensed therapists.
On some occasions, we determine that inpatient care is appropriate, on others we refer the patient back to his/her outpatient
providers. In any case, we value the outpatient provider’s clinical judgment and communicate closely during the risk assessment process to make the very best care decisions.
We contact patients’ outpatient providers at admission, during treatment and at discharge.
After discharge a copy of each patient's discharge plan
is faxed or mailed to each outpatient provider (including PCP’s)
listed on the patient's Release of Information form.
