Assessment and Referral Services

Centennial Peaks Hospital provides a level of care assessment performed by a licensed therapist or registered nurse to determine an individual’s level of functioning and treatment needs. Upon completion, a clear treatment recommendation will be provided to the individual, their family members and referring providers, as appropriate. Recommended services may include inpatient or outpatient treatment though Centennial Peaks Hospital, or in some cases, the individual may be referred to another care provider or facility. Assessments are free of charge and provided 24 hours a day, 7 days a week.

HOW TO ACCESS CARE
Please call if you are experiencing urgent mental health or addiction needs. You may also call for a family member, friend or client. Out treatment staff is available 24 hours a day, 7 days a week. Appointments are preferred to ensure the best possible service however assessments can be performed on a walk-in basis if necessary.
