Assessment and Referral Services

WHAT IS AN ASSESSMENT?

An assessment is a level of care evaluation performed by a licensed therapist who asks specific questions in order to determine patients’ level of functioning and treatment needs.  At the conclusion of the evaluation a clear treatment recommendation is provided to the patient. This information is also given to family members and referring providers as appropriate.  Recommended services may include inpatient or outpatient treatment through Centennial Peaks Hospital or, in some cases, a client may be referred to another care provider or facility.  Most frequently insurance covers the cost of this assessment.
